Under the Affordable Care Act, many Medicare beneficiaries, including those dually eligible for Medicaid, will receive improved care through programs that coordinate health care, help patients transition from hospital to their home, and make payment changes that are designed to improve the quality of health care. Many of these efforts, while focused initially on Medicare, are expected to be replicated in Medicaid and the private insurance market.
