Alabama
Filing a complaint:
You can complete a Complaint Form online or send a letter to the Consumer Affairs Section of the Attorney General’s Office. You will be asked to include your full name, address, phone number and email. Once you file your complaint, it will be available to anyone who asks for a copy.
If you send a letter, you should include a description of the complaint and include copies of all correspondence, receipts, or materials you received. Keep the originals of all materials you received, as well as a copy of the letter you send to the Consumer Affairs Section.

Alaska
Filing a Complaint:
To file a complaint, print the PDF form and complete it in clear, dark ink. You should provide your name, address, and all other requested information because incomplete forms cannot be processed. All questions should be answered in the space provided, rather than writing "see attached" as an answer. Attach copies of documents related to the complaint. Keep a copy of the complaint for your files and mail the original complaint and copies of supporting materials to the Consumer Protection Unit.
Once you file a complaint, your information will only be released if the Attorney General decides to file a lawsuit against the company. In all other cases, your information will be kept private. If you do not want to include your personal information, you can file your complaint anonymously.

Arizona
Filing a Complaint:
You can file a complaint with the Attorney General's Office. The Office also suggests that a complaint be filed with the state or federal agency in charge of regulating the organization about which you are complaining; however, because CPCs are generally not regulated by any state or federal agency, you should file your complaint directly with the Attorney General's Office.
To request a complaint form, you can contact the Attorney General's Office by phone or mail. You can also file a complaint online. You can fill out the complaint form on the computer or by hand in legible, black ink.
When filing a complaint with the office you will be asked to include your full name, address, and email. On the complaint form, you should indicate whether the office may (a) send a copy of the complaint to the CPC, (b) provide your name and number to the media in the event of an inquiry, and (c) send a copy of the complaint to another government agency for their review or investigation.
When describing what happened, present the events in the order in which they occurred; explain what was misleading to you; and, if possible, explain why it was misleading. If you have supporting documents, you should mail copies of these materials to the Consumer Protection Information and Complaints Office along with your completed complaint form.

Arkansas
Filing a Complaint:
You can file a complaint online or print out a PDF form and mail it to the Consumer Protection Division. You should include your full name, address, phone number and email. You should also include copies of supporting materials with the completed complaint form.
Once you file a complaint, the Attorney General will keep a record of it and may launch an investigation. If there's an investigation, your complaint will be kept private until the investigation is complete. After that, it will be available to anyone who asks for a copy through the Freedom of Information Act.

Filing a Complaint:
You should file a complaint with the Attorney General's Office, either online or by submitting a completed print form. If the CPC against which you are filing a complaint is licensed to provide medical services, you can file a complaint with the California Department of Consumer Affairs which responds to complaints against licensed individuals or businesses. In either case, you will be asked to include your full name, address, phone number and email. In addition, you should include copies of any supporting documents along with your completed complaint form.
Once you file your complaint the information may be sent to another government agency as required by state or federal law; may be accessed by the public through a Public Records Act request; or may be requested through a court or administrative order, a subpoena, or a search warrant. If you do not want to include your personal information, you can file your complaint anonymously.

Filing a Complaint:
You can submit a complaint online or download a complaint form and mail it to the Division of Consumer Services at the address below. The Division requires that you fill out the form completely; incomplete forms will not be processed.
If you file the complaint online, you can select "other/miscellaneous" as the subject area for your complaint. Georgia
Filing a Complaint:
You can submit a complaint by using the Consumer Complaint Form on the Consumer Affairs' website or by writing a letter to the agency. If you do not use the complaint form, your letter must set out the problem chronologically and you must include copies of relevant supporting documents.
If you use the Consumer Complaint Form, you can file this complaint online or by downloading a PDF of the form and submitting it to the Office of Consumer Affairs by mail or fax. If you submit a complaint online, you will not receive a response from the agency unless the agency needs more information from you or if the agency decides to pursue the issue on behalf of a larger group of individuals. Complaints that are faxed must be no longer than 5 pages in length, including copies attached supporting documents.
Once you file your complaint the information becomes a matter of public record and is not considered confidential. The complaint will be available to anyone who requests a copy. Your personal identifying information, however, will not become publicly available. You may also file your complaint anonymously.

Your complaint and supporting documents may be forwarded to the organization against which you are complaining, unless you check the box on the bottom of page 2 of the complaint form. Checking this box means that you are notifying the Attorney General's Office of your concerns, but that you do not want to initiate a mediation process with the organization against which you are filing a complaint.

Iowa
Filing a Complaint:
You can file a complaint by using an online or downloaded form, or by writing a letter and mailing it to the Attorney General's Consumer Protection Division. For the Division to pursue your complaint, you must include your name, address, and phone number. You should also mail copies of any documents that relate to your complaint. If you file your complaint online and follow-up by mailing in supporting materials, you should note that you have filed a complaint online.
If you do not use the complaint form, you can send a written description of your complaint to the Consumer Protection Division. Include the following in your letter: your name, address, and contact information; the name and current address and phone number of the organization against which you are complaining; and describe the problem in chronological order. In addition, include copies of all supporting documents relevant to your complaint.
Complaints are routinely sent to the person or organization against which the complaint is directed. In addition, complaints are "open records" under Iowa law that may be obtained by the public.

Massachusetts
Filing a Complaint:
You can file a complaint online or by mail. The Attorney General's Office recommends the online form as the most efficient way to file your complaint. If you have supporting materials, however, you should file your complaint by mail. To do this, fill out the online form and click "Download a Copy of your Complaint." This will launch a PDF of your complaint which you should save, print, and mail along with copies of your supporting documents. The Attorney General's Office asks that you not submit an online complaint and mail a hard copy, as this will result in duplicate submissions.
When submitting a complaint to the office you will be asked to include your full name, address, phone number, and email. Once you file the complaint it becomes a matter of public record and will be available to anyone who requests a copy. On the form you can choose whether you want the complaint to be sent to the CPC. The office may choose to send the complaint to other enforcement and regulatory agencies.

Michigan
Filing a Complaint:
You can file your complaint online or by mailing or faxing a completed complaint form to the Consumer Protection Division of the Attorney General's Office. If you submit a complaint online, you will be given a "web file number" that you should record.
The Consumer Protection Division recommends sending copies of documents that relate to your complaint. You can mail or fax copies of these supporting materials to the Consumer Protection Division along with identifying information (web file number or your name and date) so that these materials can be matched with your complaint.
Once you submit a complaint to the Attorney General's Office, the complaint becomes a matter of public record and copies can be provided to anyone who requests them. In addition, a copy of your complaint will likely be sent to the organization against which your complaint is directed.

Filing a Complaint:
You can file a complaint by mailing a completed complaint form to the Attorney General's Office. You can access the complaint form online or by calling the Attorney General's Office and requesting that the complaint form be mailed to you. Remember to sign the completed complaint form. Filing a Complaint:

Filing a Complaint:
You can submit a complaint online or by mail. If you have supporting materials that can be uploaded and provided in Word, Excel, or PDF format, you can submit your complaint electronically along with these materials. Otherwise, you should download a PDF of the Complaint Form and mail it to the Consumer Protection and Antitrust Bureau with copies of the supporting materials.

Pennsylvania
Filing a Complaint:
The complaint form can be submitted online or printed, completed, and mailed to the Bureau of Consumer Affairs. If you have supporting documentation, such as any materials you received, you should print the form and mail it in along with copies of supporting materials.
Complaints should include your name, address, and telephone number. Processing of your complaint may be delayed if complete and accurate contact information is not provided. In describing the incident, be sure to explain what happened, when it happened, and where it happened. Be specific about anything said to you, especially statements that influenced you to visit the organization.
When contacting the Bureau, always do so in writing so that your file can be kept up to date. The Bureau requests that you not call to check the status of your complaint, but to keep the office informed of additional information regarding the complaint, preferably in writing.
The information you provide in your complaint will be shared with the CPC and may be shared with other governmental and law enforcement agencies.

Filing a Complaint:
You can file a complaint using the PDF form available at the link below. You should mail this completed form, along with copies of any supporting materials, to the Office of Consumer Affairs. If the organization against which your complaint is directed is located in Fairfax County, you should mail all documents to the Fairfax office. All information provided in your complaint is available for public review.

Washington
Filing a Complaint:
You can file a complaint by submitting a form online or by mailing a completed form to the local branch of the Attorney General's Office. You can upload any supporting documents or enclose these with your mailed complaint.
For your complaint to be processed, you must agree that the Attorney General's Office has permission to send a copy of your complaint to the organization against which you are complaining. In addition, your complaint and supporting documents become public record, subject to disclosure requests.

Wisconsin
Filing a Complaint:
You can file a complaint online or by mailing a completed form to the Bureau of Consumer Protection. You should also provide copies of supporting documents. You can send scanned copies of these materials by email (to accompany your online complaint) or mail copies to the Bureau of Consumer Protection (with your hard-copy complaint form).
The complaint will likely be sent to the organization about which you are complaining. In addition, the complaint will be available for public review upon request after the Bureau of Consumer Protection's action is completed. However, you may submit your complaint anonymously.
